Effective October 1st, 2021, New York State Medicaid requires Drug Enforcement Administration-eligible Medicaid-participating healthcare providers. Basically, any healthcare provider who can make a prescription is obligated to fill & submit this form.
This regulation applies to physicians, PAs, NPs, midwives, etc.
It is noteworthy that this attestation needs to be completed even by those providers who do not have a DEA number. The form is available here:
