Four Simple Steps.
Log In
Please enter your username and password, and then click on the login button.
Submit a Request
Fill in the request form for real-time eligibility — patient's last and first name, sex, date of birth, insurance name, insurance ID number, SSN number, and eligibility date. Once all required information is entered, click "Submit Request". If any information is missing, you will not be able to submit.
Review History
Access the request history by clicking on the "History" tab. You will find a list of patients for whom requests were submitted, sortable by insurance, provider, inserted date, or personal patient information. Click "View Response" to see the response and additional details.
View Response
A new window displays the patient's personal information at the top — name, address, insurance information, and ID number. Click "Insurance Eligibility Status and Benefits Report" to view detailed health plan info including effective and termination dates, deductibles, and co-pays.
